This report has been formatted for online viewing. Printing instructions are as follows. This study is best suited with duplex printing for pages 1 through 6 and 9 through 14 separately on 8.5 inches by 11 inches paper with a portrait orientation. Pages 7 and 8 are designed for single sided printing on 11 inches by 17 inches paper in landscape orientation to be inserted between pages 6 and 9. These inserts are meant to be folded horizontally in half (print side enclosed) followed by another half fold of the right flap (bringing part of the print side back out). The title box on the right of each foldout serves as a guide for this process.
